Medical and emergency card

Fill this in for each person, then print it and cut out the card to keep in a wallet, purse or bag. The QR code holds the same text, so a phone camera can read it. What you type stays in this browser on this device. It is never sent to OrCore Prep, and it is not saved to your household account. Use Clear on a shared computer.

In an emergency call 999 (112 also works in the UK: GOV.UK). A card helps paramedics and helpers, but they may not see it: tell them about allergies and medicines if you can.

Blood group: only fill it in if you know it from a test (it is on some NHS app records and blood donor cards). Your NHS number is optional; it is on letters from the NHS and in the NHS App. Also see first aid basics and your household emergency plan.
